Reflecting back over your life in general, please select all statements that would typically apply to you. In some questions this may be every statement while in others it may be none. At the end of each question either press the OK button (if it appears) or move to the next question using the "next" button on the progress bar. At the end of all 10 questions, please press "submit" for your result.

Remember you are NOT considering any symptoms that have emerged at this stage but are identifying a baseline from which you can work through the other steps in the herbal protocol.
